Physical Therapist - Travel Contract

About the Role

A traveling Physical Therapist providing home health care, focusing on patient recovery and management of chronic conditions. The role involves evaluating functional status, developing treatment plans, delivering evidence-based interventions, documenting progress, and communicating with healthcare team members.

Responsibilities

  • Assess patient needs and develop individualized care plans.
  • Provide treatments to relieve pain and restore or maintain function.
  • Document patient progress and communicate changes to nursing and physicians.
  • Participate in on-call and weekend coverage across service areas.

Requirements

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
  • Current licensure in Massachusetts.
  • CPR certification, valid driver’s license, vehicle insurance, and access to a reliable vehicle.
  • Experience in home health, especially post-acute hospitalization or assisted living, is preferred.
  • Ability to work independently and onboard quickly.

Schedule

Evening shifts (2000-0400), guaranteeing 32 hours weekly.
