Physical Therapist - Travel Contract

Role Overview Support home health patients as a travel Physical Therapist, evaluating functional status, developing and implementing plans of care, documenting per policies, instructing patients and caregivers, and collaborating with interdisciplinary teams to achieve patient goals. The role involves travel within the service area, supervising PTAs and home health aides when permitted, and maintaining licensure and documentation standards. Qualifications & Skills Degree in Physical Therapy from an accredited program.Active state license required.Experience with home health and OASIS documentation.CPR certification; BLS may be required.Ability to evaluate, develop care plans, and report outcomes promptly.Flexible schedule for weekend and holiday coverage.Willingness to travel within the designated area. Additional Preferences Experience with electronic documentation systems like HCHB.Supervision experience of PTAs and aides.
