Physical Therapist - Travel Contract

About the role

We are seeking a traveling Physical Therapist to provide home health services in Cadillac, MI and nearby communities. This role supports patient recovery, functional independence, and safe discharges, contributing to quality outcomes in a diverse, outdoor-rich community setting.

Responsibilities

• Deliver patient-centered care to help individuals recover from injuries and manage chronic conditions
• Complete accurate and timely documentation using EMR systems like Home Care Home Base
• Develop and follow visit plans tailored to patient needs
• Coordinate with an interdisciplinary healthcare team
• Conduct evaluations and assist with discharge planning
• Meet productivity expectations while maintaining high-quality care

Requirements

• Degree in Physical Therapy from an accredited program
• Active Michigan Physical Therapy license
• BLS certification (possibly required)
• Reliable transportation and ability to travel within a one-hour radius of Cadillac, MI
• Experience with EMR systems is preferred
