Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- Home health experience
- Experience with HCHB
- Associates Degree
Responsibilities
- Performing Oasis assessments using HCHB
- Supervising PTA
- Completing admissions, recerts, 30 day assessments, and discharges
Assignment Details
- Shift schedule: Mon-Fri 8:00 am to 4:30 pm
- No on-call or weekend requirement
- Coverage areas include Logan and Todd
- Mileage reimbursed between visits, from first patient to last patient

Downtown Russellville features the Logan County Courthouse and is about 30 miles from Bowling Green, which provides additional dining, shopping, and entertainment options. Lake Malone State Park is roughly 45 miles away and offers hiking, fishing, and boating, while Mammoth Cave National Park is about 70 miles northeast for day-trip outdoor recreation.
