Physical Therapist - Travel Contract

About the Role

We are seeking a Traveling Physical Therapist to provide home health services in Cadillac, MI and nearby communities. You will help patients recover from injuries, manage chronic conditions, and collaborate within an interdisciplinary team.

Responsibilities

• Provide home health physical therapy services
• Assist patients in recovering from injuries and managing chronic conditions
• Collaborate with an interdisciplinary team
• Utilize EMR systems like Home Care Home Base for documentation and visit planning
• Perform patient evaluations and discharge planning
• Communicate effectively, including via video conferencing

Requirements

• Degree in Physical Therapy from an accredited program
• Active Michigan Physical Therapy license
• BLS certification
• Ability to travel within a one-hour radius of Cadillac, MI

Preferred Qualifications

• Experience with EMR systems, particularly Home Care Home Base
• Strong documentation and visit planning skills
