About the Role
A traveling Physical Therapist provides home health services across multiple counties, performing evaluations, Start of Care visits, recertifications, and treatments in patients' homes. The role requires reliable transportation and a valid Ohio PT license.
Responsibilities
- Perform patient evaluations, treatments, and recertifications in home settings
- Utilize HCHB platform for documentation
- Travel between patient locations within designated counties
- Maintain professional appearance in scrubs
Requirements
- Degree in Physical Therapy (Associate to Doctoral) from an accredited program
- Active Ohio PT license
- Minimum one year of home health experience
- Proficiency with HCHB documentation
- BLS certification (may be required)
Preferred Qualifications
- Lymphedema therapy experience
Schedule
- Daytime hours in a 5x8 schedule
- Guaranteed 40 hours per week
- No on-call or weekend commitments
