Physical Therapist - Travel Contract

About the role

A Traveling Physical Therapist will provide rehabilitative services at a skilled nursing facility, focusing on enhancing patient outcomes and quality of life.

Responsibilities

• Conduct patient assessments and develop individualized treatment plans
• Deliver physical therapy interventions and monitor progress
• Participate in interdisciplinary care team meetings
• Document patient care electronically and maintain accurate records
• Collaborate with team members to optimize patient rehabilitation

Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
• Active state license in Physical Therapy
• Experience in skilled nursing or long-term care preferred
• Proficiency in mobility training, fall prevention, and restorative protocols

Schedule

Monday through Friday, 8:30 am to 4:30 pm.
