About the role
Join our team as a traveling Physical Therapist in a skilled nursing facility, providing rehabilitative services to help patients recover from injuries and manage chronic conditions.
Responsibilities
- Conducting assessments, developing treatment plans, and delivering therapy to improve patient function
- Documenting patient care electronically and collaborating with interdisciplinary teams
- Participating in discharge planning and patient progress evaluations
- Provide individualized therapy services
- Maintain accurate documentation
- Support patient recovery and quality of life
- Engage in interdisciplinary care planning
Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
- Active license in Michigan
- BLS certification (may be required)
Skills
- Preferred experience in skilled nursing or long-term care, mobility training, fall prevention, and restorative protocols
- Ability to work independently and as part of a team
Schedule
Shift hours: 08:30–16:30, Monday to Friday, with approximately 35 hours weekly
