Physical Therapist - Travel Contract

About the role

Join our team as a traveling Physical Therapist in a skilled nursing facility, providing rehabilitative services to help patients recover from injuries and manage chronic conditions.

Responsibilities

• Conducting assessments, developing treatment plans, and delivering therapy to improve patient function
• Documenting patient care electronically and collaborating with interdisciplinary teams
• Participating in discharge planning and patient progress evaluations
• Provide individualized therapy services
• Maintain accurate documentation
• Support patient recovery and quality of life
• Engage in interdisciplinary care planning

Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
• Active license in Michigan
• BLS certification (may be required)

Skills

• Preferred experience in skilled nursing or long-term care, mobility training, fall prevention, and restorative protocols
• Ability to work independently and as part of a team

Schedule

Shift hours: 08:30–16:30, Monday to Friday, with approximately 35 hours weekly
