About the Role

Provide skilled evaluations, develop care plans, and deliver patient-centered physical therapy in home settings for adults with acute and chronic conditions. Collaborate with an interdisciplinary team and a manager to ensure quality care across a geographic territory. Manage a caseload within Walla Walla County and neighboring areas, traveling up to approximately 60 miles.

Responsibilities

  • Document patient progress and outcomes using electronic medical records, supporting safe discharges and functional gains.
  • Support patient independence, recovery, and community continuity of care.

Qualifications

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
  • Active Washington license required; Oregon license or willingness to obtain it is necessary.
  • At least 12 months of recent home health physical therapy experience.
  • BLS certification may be required; preferred experience with HomeCare HomeBase (HCHB).
  • Comfortable managing a geographic caseload and traveling within assigned territory.

Schedule

Work Monday through Friday, 8:00 AM to 5:00 PM, within a team setting.
