Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited institution
- Active State License in Physical Therapy
- BLS certification (may be required)
- At least 1 year of home health experience with a Medicare-certified facility
- HCHB experience

Schedule
Work weekdays, 8:00 AM to 5:00 PM, with no on-call or weekend duties.
