Travel Physical Therapist position in Horseheads, NY providing skilled therapeutic interventions to improve mobility, strength, and safety for patients in a skilled nursing facility. You will document patient progress clearly and timely, collaborate with nursing and families to coordinate care, and assist with discharge planning to support safe transitions.

Responsibilities

• Provide skilled therapeutic interventions to improve patient mobility, strength, and safety
• Document patient progress clearly and in a timely manner
• Collaborate with nursing staff and families to coordinate patient care
• Assist with discharge planning to support safe transitions

Requirements

• Current New York PT license
• Recent clinical experience in skilled nursing or rehabilitation
• 1 reference from entire work history
• Must have SNF experience and be able to hit the ground running

$1,949.00 - $2,008.00 per week
