Responsibilities

• Provide geriatric house calls to patients in their homes and senior living communities.
• Assess and treat physical and occupational therapy needs.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Document progress and communicate with patients, families, and other healthcare providers.

Requirements

• Current licensure as a Physical Therapist or Occupational Therapist.
• Experience in geriatric care and home health settings.
• Strong communication and interpersonal skills.
• Ability to travel and work in various locations.

Qualifications

• Master’s degree in Physical Therapy or Occupational Therapy.
• Passing scores on the national licensing exams.
• Excellent clinical skills and knowledge of geriatric care.
• Proficiency in Microsoft Office and electronic medical records systems.
