About the role

Deliver personalized, one-on-one care to older adults in private homes and senior living communities using the Geriatric House Calls™ model. This role offers autonomy, flexibility, and the option to incorporate telehealth and virtual care for enhanced access.

Responsibilities

• Conduct evaluations and create individualized treatment plans
• Provide evidence-based therapy to improve mobility and independence
• Document patient progress in an electronic medical record (EMR)
• Collaborate with interdisciplinary teams
• Educate patients and caregivers

Requirements

• Valid Physical Therapist (PT) license
• Degree from an accredited physical therapy program
• Basic computer skills
• CPR certification

Physical demands
