Physical Therapist (Sign-on Bonus)

About the role

We are seeking a licensed Physical Therapist to deliver personalized, one-on-one care to older adults in private residences and senior living communities. The role involves collaborating with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists, as well as educating patients, caregivers, and staff to promote safety and aging in place. Flexibility to incorporate telehealth/virtual care is available, enhancing access and care continuity.

Responsibilities

• Perform evaluations and develop individualized treatment plans
• Provide evidence-based therapy sessions to improve mobility and independence
• Document care in compliance with regulations
• Collaborate with an interdisciplinary team of healthcare professionals
• Educate patients, caregivers, and staff on safety and aging in place

Requirements

• Valid Physical Therapist license
• Degree from an accredited Physical Therapy program
• Basic EMR (Electronic Medical Records) proficiency
• Current CPR certification
