Physical Therapist (Sign-On Bonus)

About the role

We are seeking a licensed Physical Therapist to provide personalized in-home and senior living care, supporting older adults to improve mobility, independence, and overall quality of life.

Responsibilities

• Perform evaluations and develop individualized, evidence-based treatment plans.
• Deliver therapy sessions to promote wellness, mobility, and functional independence.
• Document patient progress accurately within the EMR system.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to enhance safety and aging-in-place.
• Provide telehealth and virtual care to extend access and flexibility.

Requirements

• Valid Physical Therapy license or eligibility in the practice state.
• Degree from an accredited Physical Therapy program.
• Proficiency in basic computer skills and EMR systems.
• Current CPR certification.

Skills and Physical Requirements

• Ability to exert medium physical effort, including lifting and assisting with patient transfers.
• Capability to perform CPR, reposition patients, and navigate community settings.
• Willingness to drive to patient locations and wear PPE as needed.
