Responsibilities
- Conducting initial evaluations and assessments to identify patient needs and goals
- Developing and implementing comprehensive treatment plans, including physical therapy interventions, modalities, and exercises
- Collaborating with other healthcare professionals to ensure seamless care coordination and communication
- Providing education and support to patients and their families to promote independence and self-care
- Maintaining accurate and timely records of patient progress and treatment plans
