About the role

We are looking for a dedicated Physical Therapist to join our multidisciplinary team full time in Rockford IL. The patient population is primarily 60+, commonly presenting with: Knee arthritis, Spinal pain, Balance and coordination issues (often neuropathy-related).

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Restore function, reduce pain, and prevent disability
• Perform manual therapy, therapeutic exercise, and modalities
• Collaborate with other providers on patient care
• Track progress and adjust treatment plans as needed
• Educate patients and families on recovery and home programs
• Maintain accurate documentation

Requirements

• Graduation from an accredited Physical Therapy program
• Physical Therapy license to practice in IL

Schedule & Benefits

• Monday–Friday schedule
• Early finish on Fridays (done by 1 PM)
