Physical Therapist – Rehabilitation Department
Position Summary
As a Physical Therapist at Blue Mountain Hospital District (BMHD) you will join the rehabilitation department team that includes SLP, occupational and physical therapy services. This position is primarily in the Home-Based setting, though there is the ability to work partial hours in inpatient care.
Essential Functions & Responsibilities
Inpatient/Home-Based Position
- Performs accurate and comprehensive assessments
- Completes OASIS data sets for “therapy only” cases
- Ensures that changes in patients’ clinical status are communicated to other healthcare team members and HHA office staff verbally and in writing in a timely manner
- Ensures that physician orders are communicated to other healthcare team members in a timely, accurate, and thorough manner
- Carefully coordinates care planning and delivery with the physician, patient and family/caregivers, other healthcare team members and HHA staff to facilitate optimal patient outcomes
- Assesses the patient for the need for durable medical equipment as well as other adaptive equipment needs
- Develops appropriate home exercise programs tailored to the patient’s clinical status, needs and age
- Provide supervision and mentorship to PT assistants
- Submits verification of completed visits in accordance with HHA policy
- Maintains current knowledge of community resources
- Identifies and accesses appropriate external and internal resources to meet patient and/or family needs
- Must have ability to work independently and willing to serve our patients, county-wide
- Must have time management skills, able to set up own appointments with patients
- May have supervisory responsibilities for a Physical Therapy Assistant
- Maintains self-motivation and sustains a high level of momentum
- Able to successfully prioritize when faced with requests, demands and deadlines
- Knowledge of customer service concepts
- Maintains a sensitive, fair and impartial approach to dealing with team members
- Supports and maintains a culture of safety, quality and positive patient experience
- Working knowledge of relevant regulatory requirements CMS, National Patient Safety Goals (NPSGs), and continued readiness related to healthcare or hospital standards
- Able to prioritize, highly organized
Inpatient/Outpatient Position
- Performs thorough physical therapy evaluation, evidence-based plan or care, and skilled interventions, performing re-assessments and required paperwork in a timely manner
- Administers physical therapy interventions specific to the patient’s need, may include manual therapy, neuromuscular re-education, gait training, therapeutic activities, therapeutic exercises for example to achieve functional goals set at evaluation
- Affirmation of continuation of therapeutic plan following discharge by designing home exercise programs, instruction to patients, families, and caregivers in physical therapy recommendations, recommending and/or providing assistive equipment
- Carefully coordinates care with physician referrals and directs care independently with self-referrals
- Participation easily and effectively amongst the team of medical providers, understanding and knowing the role of inpatient, transitional care, and outpatient services
- May be required to direct treatment given by a physical therapy assistant
- Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, attending webinars, establishing professional networks, participates in professional organizations
- Any additional duties pertaining to the daily operations of the Rehabilitation Services Department as assigned by the Rehabilitation Services Manager

Education and Experience Requirements
- Graduate of an accredited school of physical therapy
- For Medicare certified agencies: Graduate of a physical therapy curriculum approved by The American Physical Therapy Association, or The Committee on Allied Health Education and Accreditation of the American Medical Association, or The Council on Medical Education of the American Medical Association and the American Physical Therapy Association
- Doctorate of Physical Medicine preferred
- Strong physical therapy skills for the neurologically impaired patient population
- Experience in vestibular rehab preferred
- Experience in cardiac rehab preferred
- Experience treating chronic pain preferred
Certification/Licensure
- Basic Life Support (BLS) certified by American Heart Association (AHA)
- PT License in good standing with State of Oregon
- Valid Driver’s License
