Provides a full range of physical therapy services including assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment. Demonstrates accountability for program development, quality improvement, problem-solving, and productivity enhancement.
Qualifications
- Graduate of an accredited four-year Bachelor’s degree program in Physical Therapy (or foreign equivalent).
- Current licensure in the state of practice.
- Current BLS Certification.
Experience
Experience in working with patients to assist in the rehabilitation of disabled or injured individuals across the lifespan.
Responsibilities
- Communicates and demonstrates a professional image/attitude for patients, families, clients, coworkers, and others.
- Conducts initial assessments using appropriate tools and adaptive equipment based on the patient’s age and developmental level. Completes and documents assessments in the medical record per policy.
- Develops individualized treatment plans with measurable short- and long-term goals tied to functional deficits. Selects interventions appropriate for the patient’s age, cognitive abilities, and developmental level, including administering medication as ordered.
- Evaluates patient response to treatment, revises goals as needed, and updates the treatment plan. Provides ongoing updates to patients and families regarding status, program planning, and discharge plans.
- Documents patient care, including progress, skilled intervention needs, and clinical recommendations. Completes daily progress notes, outpatient notes (at least every 10 visits or per state practice act), and discharge summaries per policy. Ensures appropriate charge capture for services provided.
- Provides patient and family education on tasks and resources to support continued functional independence. Integrates family members into treatment, education, and discharge planning for safe transitions and ongoing care. Measures education effectiveness through peer input, family feedback, and patient satisfaction surveys.
- Participates in multidisciplinary discharge planning, including determining the appropriate next level of care.
- Engages in continuing education and professional development. Attends and presents at departmental in-service sessions and external professional growth activities, sharing knowledge with the team.
- Adheres to federal, state, and organizational policies, including professional codes, employee handbooks, and unit-specific procedures.
- Meets established productivity standards in a fiscally responsible manner.
- Performs other duties as assigned, including all equipment and tasks within the Physical Therapy Department as outlined in the Competency Checklist.
Pay
The contracted rate of pay for this per diem position is $68.00 per hour.
