About the Role
Join a dedicated healthcare team as a Physical Therapist (PT) in a home health setting, providing skilled rehabilitative care to clients in their homes.
Responsibilities
- Perform diagnostic tests
- Develop and revise individualized care plans
- Implement therapeutic exercises, gait and balance training
- Provide wound care interventions
- Educate clients and families on rehabilitative activities
- Accurately document patient observations, interventions, and evaluations
Requirements
- Valid Hawaii PT license
- Graduation from an accredited Physical Therapy program
- Solid computer skills; EMR experience preferred
