Physical Therapist (PT) up to $55/hr!

Position Overview Seeking an experienced, compassionate Physical Therapist with long-term care expertise to join a dynamic rehab team serving adult and geriatric residents in post-acute settings. This full-time role involves evaluating, diagnosing, and treating residents with mobility, strength, balance, gait, and pain impairments in SNF/LTC environments. The therapist develops individualized treatment plans collaboratively with interdisciplinary staff, documents care accurately, and educates residents, families, and staff on therapeutic exercises, fall prevention, and safety. Active participation in care conferences, quality improvement initiatives, and program development is expected. Qualifications & Skills Unrestricted Physical Therapist license in the practice state (or eligibility).Experience in skilled nursing, post-acute rehab, or long-term care preferred.Excellent communication, documentation, and collaborative skills; resident-centered approach. Key Duties Assess and treat residents with mobility and pain issues.Create and implement goal-oriented treatment plans.Maintain accurate documentation per standards.Educate residents, families, and staff on safety and mobility aids.Participate in interdisciplinary care and quality initiatives.
