About the Role

Seeking an experienced, compassionate Physical Therapist with long-term care expertise to join a dynamic rehab team serving adult and geriatric residents in post-acute settings. This full-time role involves evaluating, diagnosing, and treating residents with mobility, strength, balance, gait, and pain impairments in SNF/LTC environments.

Responsibilities

• Assess and treat residents with mobility and pain issues.
• Create and implement goal-oriented, individualized treatment plans collaboratively with interdisciplinary staff.
• Maintain accurate documentation per regulatory and facility standards.
• Educate residents, families, and staff on therapeutic exercises, fall prevention, safety, and mobility aids.
• Participate in interdisciplinary care conferences, quality improvement initiatives, and program development.

Requirements

• Unrestricted Physical Therapist license in the practice state (or eligibility).
• Experience in skilled nursing, post-acute rehab, or long-term care preferred.
• Excellent communication, documentation, and collaborative skills.
• Resident-centered approach.
