About the role
The Physical Therapist provides direct rehabilitation care in a skilled nursing facility setting, focusing on helping patients achieve functional progress and improve quality of life. They collaborate with an interdisciplinary team to deliver long-term care and short-term rehabilitation for a diverse patient population with orthopedic, neurological, and cardiopulmonary needs.
Responsibilities
- Implement and modify individualized treatment plans based on physician referrals
- Assess patient needs, establish goals, and evaluate progress
- Supervise and evaluate PT assistants, rehab aides, and students
- Document patient care accurately and timely using therapy software
- Ensure adherence to safety, sanitation, and confidentiality standards
- Participate in community health initiatives and interdisciplinary communication
- Assist in patient evacuation and provide excellent customer service
Qualifications
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
- Active state licensure/registration and maintenance of licensure
- CPR certification (obtained upon hire or during orientation)
Skills
- Strong communication, body mechanics, and decision-making skills
- Proficiency in Microsoft Word, Excel, and email
- Ability to work productively within a team and uphold professional standards
