About the role

Continuum Therapy Partners is seeking a Physical Therapist for a PRN opportunity in Fort Worth, TX. The position is at a supportive, collaborative skilled nursing facility where the therapist's expertise makes a daily impact.

Responsibilities

• Develop and implement individualized treatment plans based on patient needs
• Perform therapeutic exercises and treatments to improve patient function
• Evaluate patient progress and adjust treatment plans as necessary
• Collaborate with interdisciplinary team members to ensure comprehensive patient care
• Document patient progress and treatment sessions accurately

Requirements

• Current state licensure as a Physical Therapist
• Experience in a skilled nursing facility setting preferred
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program
• Passing scores on the National Physical Therapy Examination (NPTE)
