About the role

Middletown Health & Rehab (formerly Laurels of Middletown) is a supportive, collaborative skilled nursing facility located in Middletown, OH. Continuum Therapy Partners is offering a PRN opportunity for a licensed Physical Therapist / PT.

Responsibilities

• Provide physical therapy services to patients in accordance with their treatment plans.
• Assess patient needs and develop individualized treatment programs.
• Document patient progress and communicate with interdisciplinary team members.
• Adapt treatment methods based on patient feedback and outcomes.

Requirements

• Licensed Physical Therapist in the state of Ohio.
• Current CPR certification.
• Experience in acute care settings preferred.

Qualifications

• Strong interpersonal skills.
• Ability to work independently and as part of a team.
• Passion for rehabilitation and patient care.

Skills

• Skilled in assessment and treatment of orthopedic conditions.
• Proficient in using rehabilitation equipment and technology.
