About the Role
Provide home-based physical therapy to patients recovering from illness or injury in various locations across New Jersey.
Responsibilities
- Collaborate with a multidisciplinary home health team to develop and implement personalized care plans.
- Observe patient movements, conduct assessments, document progress, and adjust care as needed.
- Assist patients with exercises and hands-on therapy to improve mobility and manage pain.
- Educate patients and families on care plans, exercises, and safety protocols.
Requirements
- Graduate of an accredited Physical Therapist program.
- Active PT license in New Jersey.
- Minimum of 2 years of experience, preferably in home healthcare.
- CPR certification.
- Knowledge of home health regulations.
