About the role

A Licensed Physical Therapist responsible for developing and implementing individualized treatment plans for patients of all ages in a community-focused setting.

Responsibilities

• Evaluate patients' physical conditions and establish treatment goals based on initial assessments.
• Develop and supervise personalized therapeutic exercise and treatment programs.
• Utilize modalities such as ultrasound, electrical stimulation, and manual techniques.
• Instruct patients and staff on exercises and movement principles for rehabilitation.
• Document progress, treatment outcomes, and communicate with referring providers.
• Supervise Physical Therapy Assistants, ensuring compliance with regulations.
• Participate in ongoing professional development and educational activities.

Qualifications & Skills

• Licensed Physical Therapist (PT) with valid licensure in Illinois.
• Bachelor’s Degree in Physical Therapy.
• Current Basic Life Support (BLS) certification.
• Strong clinical assessment, instructional, and documentation skills.
• Ability to collaborate with multidisciplinary teams and support staff.
