Physical Therapist (PT) Outpatient - PRN

About the Role

We are hiring a resident-centered Physical Therapist to join our team at Ann's Choice by Erickson Senior Living. All residents live within our award-winning campus, creating an environment where you can be part of their journey throughout their continuum of care. Our Physical Therapists provide a holistic approach, supporting residents' physical, spiritual, emotional, and clinical needs. They implement residents’ personal care plans, focusing on strengths, preferences, and preferred routines, while providing comfort, companionship, and belonging.

Our communities offer levels of care and include specialty programs such as urinary incontinence, vestibular rehab, home safety assessment, movement disorders, and low vision. All clients live within our community, eliminating additional travel.

Responsibilities

• Implement residents’ personal care plans with a focus on their strengths, preferences, and routines.
• Provide a holistic approach addressing physical, spiritual, emotional, and clinical needs.
• Complete documentation to meet reimbursement and regulatory requirements.

Requirements

• Valid and current state Physical Therapy license.
• Degree and training must meet current Medicare and state requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.
• Ability to adhere to specific state regulations, which take precedence over general job description requirements.

Pay

$55/hour (PRN/Per Diem).
