Physical Therapist (PT)
Job Responsibilities
- Completes daily progress notes reflecting patients' responses to treatment, actual performance, and an assessment of status relevant to the treatment plan, including eventual discharge needs and treatment summaries.
- Contacts families, physicians, or other healthcare team members to facilitate effective treatment plans in anticipation of special needs of the patients or families.
- Provides educational materials and methods appropriate to the patient's level of understanding and motivation, including families/caregivers in education and training.
- Demonstrates collaboration in communication with team members and openness to feedback and receptivity to change.
- Gives appropriate instruction and supervision to assistive personnel consistent with program/clinic policies and regulatory requirements.
- Applies knowledge of therapeutic techniques during evaluations and throughout treatment, adjusting treatment plans regularly and appropriately as patients' status changes.
- Demonstrates current knowledge of the application of therapeutic techniques and evaluation methods.
- Assumes complete responsibility over the course of Physical Therapy patient management from initial contact to discharge, focusing on functional progress, safety, and treatment outcomes.
- Develops goals and treatment plans prioritized based on input from patients, families, and healthcare team members, reflecting evaluation findings.
- Formulates comprehensive treatment plans to improve the functional status of patients and address reasons for referrals.
Minimum Qualifications
- Education: Bachelor of Science in Physical Therapy
- Licensure: Physical Therapist upon start (VA licensure or compact privileges)
- Certification: BLS through the American Heart Association upon start
Preferred Qualifications
- Lymphedema & oncology experience
- Lymphedema certification
