Physical Therapist (PT) - Outpatient Orthopedic

About the role

Join a dynamic outpatient physical therapy team specializing in in-home rehabilitation services for older adults. You will assess, develop, and implement personalized therapy programs aimed at restoring and enhancing patient mobility, strength, and independence.

Responsibilities

• Provide outpatient rehabilitation in patients' homes or facilities
• Assess and develop individualized treatment plans
• Educate patients and caregivers for independence and safety
• Coordinate with other healthcare professionals and make referrals
• Supervise support staff as needed
• Maintain timely documentation and adhere to safety standards
• Perform therapeutic exercises, manual therapy, and functional training
• Design and fit adaptive devices and provide mobility assessments
• Promote health, wellness, and fall prevention programs

Requirements

• Valid state license in Physical Therapy
• Master’s degree in Physical Therapy or equivalent experience
• Reliable transportation

• Proficiency in therapeutic techniques
• Adaptive device fitting

Pay

Competitive hourly rate of $55–$62.
