About the Role

Evolve Therapy Services is seeking a compassionate Physical Therapist to provide in-house therapy services at a Skilled Nursing Facility in Freeport, IL. This PRN position involves planning, organizing, developing, and directing Physical Therapy services to ensure the highest quality of resident care in accordance with federal, state, and local standards.

Responsibilities

• Provide physical therapy services to residents following established standards, company procedures, and productivity standards.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
• Conduct regular screenings of residents to determine the need for intervention or treatment.
• Evaluate residents to gather data for treatment planning and implementation, including specialized evaluations as needed.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and interdisciplinary team members.
• Complete compliant and medically necessary documentation in NetHealth EMR systems, adhering to CMS expectations.
• Perform additional duties as assigned to support the organization’s goals.

Requirements

• Minimum of a Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in the state(s) of practice.
• Commitment to ongoing continuing education and professional development to maintain licensure and certification.
