Physical Therapist (PT) In House PRN

About the Role

Evolve Therapy Services is hiring compassionate therapists for our partner located in Altoona, PA. This PRN position assists in planning, organizing, developing, and directing Physical Therapy services in an in-house Skilled Nursing Facility, ensuring the highest degree of quality resident care in accordance with current federal, state, and local standards and regulations.

Responsibilities

• Provide physical therapy services to residents following established standards, company procedures, and therapy department productivity standards.
• Adhere to company procedures for documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
• Conduct regular screenings of residents to determine the need for intervention or treatment.
• Evaluate residents to gather data necessary for treatment planning and implementation, including specialized evaluations as indicated.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, responsible parties, and interdisciplinary team members.
• Complete compliant and medically necessary documentation in NetHealth EMR systems, adhering to CMS expectations.
• Perform all other duties required to effectively discharge the responsibilities of the position.

Requirements

• Minimum of a Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in the state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
