- Per Visit | Earn up to $103,500 annually based on productivity
- Sign-On Bonus: $5,000

About the role
You will provide one-on-one care in patients’ homes, helping them regain strength, mobility, and confidence in their daily lives.
Responsibilities
- Conduct in-home evaluations, start of care, and follow-up visits
- Develop and implement individualized treatment plans
- Improve strength, balance, mobility, and functional independence
- Educate patients and caregivers on safety and equipment use
- Document visits using WellSky Kinnser EMR
- Collaborate with physicians, RNs, and the care team
Requirements
- Active Arizona Physical Therapist license
- CPR certification (AHA)
- Minimum 1 year of clinical experience (home health preferred)
- Strong clinical judgment and time management
- Reliable transportation, valid driver’s license, and insurance
- Strong communication and a patient-first approach
