About the Role

As a Home Health Physical Therapist, you are a critical part of the care team - helping patients safely recover, regain mobility, and maintain independence where they feel most comfortable: at home. This is a highly autonomous, relationship-driven role where you can truly see the impact of your work every day.

What You'll Do

  • Deliver Patient-Centered Care: Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential; develop and implement individualized care plans in collaboration with physicians and the care team; provide hands-on therapy to improve mobility, strength, balance, and independence; educate patients and caregivers on treatment plans, exercises, and safety strategies
  • Drive Outcomes & Recovery: Monitor patient progress and adjust treatment plans as needed; support safe transitions of care and contribute to discharge planning; identify additional patient needs and collaborate with interdisciplinary teams
  • Collaborate & Communicate: Partner closely with nurses, therapists, and physicians to ensure coordinated care; document patient care and progress accurately and timely within the EMR; participate in case conferences and team meetings
  • Lead & Support: Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines; provide guidance and education to caregivers and support staff

What You Bring

  • Graduate of an accredited Physical Therapy program (DPT or Master's)
  • Current PT license in the state of practice
  • Minimum 1–2 years of experience (home health experience preferred)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Current CPR certification
  • Valid driver's license, reliable transportation, and auto insurance
