About the role
Seeking a dedicated Physical Therapist for a full-time, non-remote role in home health coverage within Warren & Claiborne Counties.
Responsibilities
- Providing patient evaluations and developing individualized treatment plans in collaboration with physicians and team members.
- Performing treatments, educating, advising, and counseling patients and their families regarding care and progress.
- Maintaining accurate documentation of assessments, treatments, and patient progress in accordance with policies and standards.
- Communicating patient status and treatment plans effectively with interdisciplinary team members.
- Meeting agency productivity standards and optimizing work hours.
Qualifications
- Master's degree in physical therapy from an accredited program (CAPTE approved).
- Minimum of nine months of experience as a physical therapist.
- Licensed within the state of operation and compliant with professional standards.
- Current CPR certification and valid driver's license for travel to patient locations.
