Physical Therapist | PT - Full-Time | Part-Time | PRN

About the role

Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team at Sauk County Health Care Center in Reedsburg, Wisconsin. This opportunity offers Full-Time, Part-Time, and PRN scheduling options for a licensed Physical Therapist who enjoys providing meaningful, patient-centered care in a supportive skilled nursing environment.

As a Physical Therapist (PT), you will work with adult and geriatric patients to improve mobility, strength, balance, endurance, and functional independence while collaborating with an interdisciplinary rehabilitation team focused on quality outcomes.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional limitations.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve movement, independence, and quality of life.
• Perform therapeutic exercises, gait training, mobility training, and functional activities.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on mobility strategies, exercises, and safety techniques.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Wisconsin Physical Therapist (PT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for providing high-quality, patient-centered rehabilitation care.
