Physical Therapist - PT - Full Time

We are seeking a Physical Therapist (PT) to join our Rehab Team at The Community at Rockhill in Sellersville, PA (SNF) and Souderton Mennonite Home in Souderton, PA. This in-person, on-site role will primarily cover Rockhill Mennonite (75% of the time) but may require floating to Souderton or nearby senior communities (25% of the time). Some telehealth options may be available for Souderton if travel is prohibitive.

About the role

This is a full-time position where you will establish appropriate goals, creatively execute treatment plans, and supervise Physical Therapist Assistants. As part of our interdisciplinary team, you will help patients achieve critical outcomes while maintaining productivity standards and completing required documentation.

Responsibilities

• Establish and implement patient treatment goals and plans
• Supervise Physical Therapist Assistants
• Maintain productivity standards and complete required documentation
• Collaborate with an interdisciplinary team to achieve patient outcomes
• Act as a trusted partner to the community served

Requirements

• Graduate of an accredited school for Physical Therapists
• Valid or pending state license (for newer graduates)
• Experience in acute care, long-term care, and/or rehabilitation is a plus but not required
• Nethealth experience is a plus
