About the role

Always Best Care Senior Services is a Medicare-certified Home Health agency serving communities across Oʻahu. Our mission is to help seniors remain safe, independent, and comfortable in their homes by providing exceptional home health services focused on improving mobility, reducing fall risk, and enhancing quality of life. We are currently seeking a compassionate, motivated, and licensed Physical Therapist (PT) to join our growing clinical team.

Compensation

• $80–$85 per patient visit
• $200 per Start of Care (SOC) visit
• Full-Time Position
• Flexible scheduling and the opportunity to build your own caseload

What You'll Do

• Provide skilled physical therapy services in patients' homes
• Perform comprehensive initial evaluations, reassessments, and fall risk assessments
• Develop individualized treatment plans to improve strength, mobility, balance, and functional independence
• Educate patients and caregivers on home exercise programs and injury prevention
• Collaborate with nurses, physicians, and the interdisciplinary care team to ensure quality patient outcomes
• Complete accurate, timely, and compliant clinical documentation

Qualifications

• Current Hawaiʻi Physical Therapist (PT) license in good standing
• Minimum of one year of clinical experience preferred
• Home Health experience is a plus, but not required
• Strong clinical judgment, communication, and documentation skills
• Valid driver's license, reliable vehicle, and current auto insurance
• Ability to travel throughout Oʻahu to provide in-home patient care
