About the role

FeldCare Connects is seeking a self-motivated Independent Contractor Physical Therapist to deliver premier excellence of care to adults of all ages in the Agoura Hills area. The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs to promote the ability to move, reduce pain, restore function, and prevent disability. Based on this assessment, the Physical Therapist determines a treatment plan, executes interventions to improve patient well-being, and monitors progress effectively.

Responsibilities

  • Assess and evaluate patient care needs related to mobility, pain reduction, function restoration, and disability prevention.
  • Develop and implement individualized treatment plans based on patient assessments.
  • Provide a variety of physical therapy treatments tailored to patient needs.
  • Monitor and document patient progress with accuracy and discretion.
  • Modify treatment plans as needed and report adjustments to ensure appropriate patient care.
  • Communicate effectively with patients, caregivers, and healthcare providers both orally and in writing.
  • Maintain thorough and accurate records of patient care and progress.

Requirements

  • Remarkable knowledge of physical therapy principles, theories, and methods in a rehabilitation environment.
  • Exceptional ability to understand the role of therapy in the context of the patient’s age, needs, and environment.
  • Proven sound clinical reasoning in treatment planning, implementation, and progress monitoring.
  • Ability to recognize when patient protocols or treatment plans require modification.

Qualifications

  • Physical Therapist license and registration by the state.
  • Completion of a Physical Therapist curriculum approved by the American Physical Therapy Association, the Council on Medical Education and Hospitals of the American Medical Association, or the Council on Medical Education of the American Medical Association.
  • Enthusiasm and ability to work with adults of all ages.
  • Bilingual skills are a plus.
