Concierge Home Care is hiring a Home Health Physical Therapist (PT) in Lakeland, FL, serving Polk County with territories designed to minimize drive time through clustered scheduling. The position offers 1:1 patient care in the home, flexible scheduling, and autonomy.

• Pay‑per‑visit model: $70+ per visit + Medicare differential pay

Your Role as a Home Health Physical Therapist

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology

Qualifications

• Active, unencumbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance
