Physical Therapist (PT) - Center Based

Position Overview

The Physical Therapist (PT) plans, conducts, and implements individualized treatment programs to restore and maintain patient well-being, adhering to confidentiality and professional standards.

Key Responsibilities

• Provide high-quality physical therapy services following physician prescriptions.
• Develop written evaluations outlining patient needs, goals, and progress in collaboration with healthcare teams.
• Coordinate services, discharge planning, and communicate effectively with staff, patients, and families.
• Serve as a clinical resource and supervise other personnel as appropriate.
• Complete assessments such as OASIS where permitted by regulation.

Qualifications & Skills

• Bachelor of Science in Physical Therapy from an approved program.
• Current, unrestricted state license and CPR certification.
• Proficiency in assessments, documentation, and compliance with policies.
• Preferred: at least one year of recent experience in acute care or rehab, pediatric experience is a plus.
• Strong attention to detail, organizational, problem-solving, and communication skills.

Physical & Environmental Requirements

• Ability to lift up to 50 lbs, perform frequent standing, bending, and reaching.
• Effective in responding to emergencies, with visual, hearing, and sensory acuity.
• Capable of traveling, repositioning patients, and working in diverse healthcare settings, including exposure to infectious conditions.

Schedule

This role involves working primarily during daytime hours, with no nights or weekends, and may include supervision duties.
