Physical Therapist PT

About the role

The Physical Therapist (PT) evaluates, plans, and implements individualized treatment programs for residents in a skilled nursing facility. The role focuses on restoring function, improving mobility, relieving pain, and preventing or limiting permanent physical disabilities while ensuring high-quality care delivery and compliance with regulatory and reimbursement guidelines.

Responsibilities

• Perform comprehensive evaluations of residents’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement individualized treatment plans based on clinical findings and physician orders.
• Provide therapeutic interventions including exercise, manual therapy, gait training, balance training, and modalities.
• Supervise and direct Physical Therapist Assistants (PTAs) and rehabilitation aides.
• Monitor resident progress and modify treatment plans as needed to achieve optimal outcomes.
• Complete accurate and timely documentation, including evaluations, progress notes, recertifications, and discharge summaries.
• Ensure compliance with Medicare (Part A & B), PDPM guidelines, and state/federal regulations.
• Participate in interdisciplinary team meetings, care planning, and discharge planning.
• Educate residents, families, and caregivers on treatment plans, safety, and home exercise programs.
• Maintain productivity standards while delivering quality patient care.
• Ensure a safe and effective therapy environment.

Requirements

• Doctor of Physical Therapy (DPT) or Bachelor’s/Master’s degree in Physical Therapy from an accredited program.
• Current Physical Therapist license in the state of practice.
• Previous experience in a skilled nursing facility or geriatric setting preferred.
• Strong knowledge of rehabilitation techniques, equipment, and safety protocols.
• Familiarity with Medicare regulations, PDPM, and documentation requirements.
• Excellent communication, clinical reasoning, and interpersonal skills.
• Ability to supervise and mentor support staff.
