About the Role

The Physical Therapist (PT) provides direct rehabilitation care to patients in a skilled nursing facility, supporting long-term care and short-term rehabilitation needs.

Responsibilities

  • Implement and modify individualized treatment plans to improve patient function and quality of life.
  • Supervise and evaluate care provided by PTAs, rehab aides, and students.
  • Chart patient progress accurately and utilize therapy software effectively.
  • Collaborate with an interdisciplinary team to deliver patient-centered care.
  • Assist in emergency procedures and maintain safety, confidentiality, and professionalism.
  • Participate in community health projects and maintain good communication with patients and families.

Requirements

  • Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT).
  • Current license or registration in applicable state, with active standing.
  • Preferably 1+ year of experience; new graduates are welcome.
  • CPR certification required.
  • Proficient in Microsoft Office, with excellent communication and teamwork skills.
  • Knowledge of post-acute care regulations and good body mechanics.
  • Ability to make independent decisions, demonstrate professionalism, adhere to policies, and maintain confidentiality.

Physical Demands

The role involves physical activity, including sitting, standing, lifting, and moving throughout work hours.
