About the Role
The Physical Therapist (PT) provides direct rehabilitation care to patients in a skilled nursing facility, supporting long-term care and short-term rehabilitation needs.
Responsibilities
- Implement and modify individualized treatment plans to improve patient function and quality of life.
- Supervise and evaluate care provided by PTAs, rehab aides, and students.
- Chart patient progress accurately and utilize therapy software effectively.
- Collaborate with an interdisciplinary team to deliver patient-centered care.
- Assist in emergency procedures and maintain safety, confidentiality, and professionalism.
- Participate in community health projects and maintain good communication with patients and families.
Requirements
- Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT).
- Current license or registration in applicable state, with active standing.
- Preferably 1+ year of experience; new graduates are welcome.
- CPR certification required.
- Proficient in Microsoft Office, with excellent communication and teamwork skills.
- Knowledge of post-acute care regulations and good body mechanics.
- Ability to make independent decisions, demonstrate professionalism, adhere to policies, and maintain confidentiality.
