About the role

Physical Therapist responsible for evaluating, developing, and implementing individualized treatment programs for patients to promote health and achieve therapy goals.

Responsibilities

• Assess patients through data collection from records, interviews, and observations.
• Design and modify treatment plans based on ongoing evaluations.
• Provide direct therapy services across various settings, aligning with medical and nursing care plans.
• Document evaluations, treatment plans, and progress notes accurately.
• Communicate effectively with patients, families, physicians, and interdisciplinary team members.
• Build rapport and collaborate with team members and families during treatment.
• Engage in professional development through educational programs and workshops.

Qualifications & Skills

• Active state license as a Physical Therapist at hire.
• Visual acuity and effective communication skills.
• Preferred 1-3 years of licensed physical therapy experience.
• Ability to work the specified hours and adhere to safety and compliance standards.
