About the role
Pine Ridge Rehabilitation & Healthcare Center is a skilled nursing and long-term care facility located in Pagosa Springs, CO. We are currently seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team.
Responsibilities
- Provide physical therapy services to residents in a skilled nursing and long-term care setting
- Complete accurate and timely documentation
- Develop and implement individualized therapy treatment plans
- Collaborate with interdisciplinary healthcare teams to support resident progress
- Follow all facility policies and procedures
- Ensure compliance with all state and federal regulations and therapy standards
- Promote resident independence, mobility, and overall quality of life
Requirements
- Current Physical Therapist (PT) license in Colorado or a compact state
- Compassionate and resident-focused professional
- Strong communication and teamwork skills
- Organized, dependable, and motivated
- Passionate about delivering high-quality rehabilitation services
Qualifications
- Master’s degree in Physical Therapy from an accredited program
- Experience in a skilled nursing or long-term care setting preferred
- Knowledge of Medicare/Medicaid regulations and reimbursement processes
Skills
- Excellent interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficient in Microsoft Office applications
- Strong organizational and time management skills
