Physical Therapist (PT)

About the role

Piedmont Post Acute is an 88-bed skilled nursing facility in Piedmont, South Carolina. We seek a compassionate Physical Therapist to join our rehab team, helping residents achieve independence.

Responsibilities

• Evaluate and treat residents to improve mobility, strength, and overall function
• Develop and implement individualized treatment plans
• Provide therapeutic interventions to enhance mobility, strength, and balance
• Maintain accurate and timely documentation of patient progress
• Collaborate with interdisciplinary team members to ensure comprehensive care
• Educate residents and families on treatment plans and goals

Requirements

• Active, unencumbered Physical Therapist (PT) license in South Carolina
• Education in Physical Therapy (Bachelor’s, Master’s, or Doctorate)
• Experience in long-term care or rehab settings preferred
• Familiarity with PointClickCare (PCC) and Casamba
• Strong communication and clinical skills
