Physical Therapist (PT)

Kadima Health and Nursing at Pottstown is seeking a Physical Therapist to join our team. Our mission is to offer quality care to our residents, providing a loving and compassionate environment while attending to their physical, spiritual, and emotional needs. The Physical Therapist will uphold facility core values in administering department services and fulfilling therapy-related resident care needs.

Responsibilities

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Ensure all treatment is delivered in accordance with an established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Measure and record patient’s motor function, strength, and muscle performance.
• Monitor patient responses to treatments and make modifications during treatment sessions as necessary.
• Complete documentation in accordance with company policy and state/federal regulations.

Requirements

• Current License as a Physical Therapist in the State of PA (must be in good standing).
• SNF/Long-term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.
