Pay
The evaluation visit rate for this role ranges from $65 - $85 per visit. Final compensation is based on experience, qualifications, and internal equity. This position is paid on a per-visit basis.

Requirements
- Active Physical Therapist license in good standing
- BLS CPR certification
- Reliable transportation to travel between patient homes
- Basic comfort with technology, including email, texting, and EMR systems (training provided)
