About the role

Flagstar Rehab is staffing Physical Therapists for state-of-the-art Skilled Nursing Facilities (SNFs) throughout NYC and Long Island. As part of a diverse, multi-disciplinary team, you will perform evaluations, treatments, and patient/family education to improve patients’ physicality and cognition, positively impacting their daily functioning.

Flagstar Rehab offers the choice of direct employment with our agency or independent contracting—we can assist you in setting up a corporation free of charge.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Deliver quality therapy services in an SNF setting for patients with TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress using innovative EMR software.

Requirements

• Current NYS Physical Therapist License and Registration.
• CPR Certification.
• EMR competence.
• Meets all state and federal requirements for employment in a nursing facility.
• Malpractice/liability insurance (if applicable).

Pay

Hourly pay rate: $46 (W-2), $57 (1099).
