About the Role

As a Physical Therapist (PT), you will have the opportunity to make a meaningful difference in the lives of our residents. Your primary responsibilities will include:

• Conducting initial and ongoing assessments of residents' physical abilities and developing individualized treatment plans
• Providing physical therapy services to residents in our state-of-the-art therapy gym and in their residences
• Collaborating with other healthcare professionals to ensure comprehensive care and achieve optimal outcomes
• Educating and empowering residents and their families to take an active role in their care and recovery
• Documenting patient progress and communicating with healthcare teams to coordinate care
