Physical Therapist (PT)

About the Role

Physical therapist position for an in-house Skilled Nursing Facility (SNF) focused on planning, organizing, developing, and directing physical therapy services. This role ensures the highest quality of resident care in accordance with federal, state, and local standards, under the direction of the Director of Rehabilitation (DOR), Administrator, or Regional support team.

Responsibilities

• Provide physical therapy services to residents following established standards, company procedures, and therapy department protocols.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
• Conduct regular screenings of residents to determine the need for intervention or treatment.
• Evaluate residents to gather data for treatment planning and implementation.
• Perform specialized evaluations as needed.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, responsible parties, and interdisciplinary team members.
• Complete compliant and medically necessary documentation in NetHealth EMR systems, adhering to CMS expectations.

Requirements

• Minimum of a Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in the state(s) of practice.
• Commitment to ongoing continuing education and professional development to maintain licensure and certification.
